DIRECTIONS FOR COMPLETING AND SUBMITTING
THE TITLE I, PART D, SUBPART 1
APPLICATION AND BUDGET
YEARLY APPLICATION FOR FUNDING
(2015-2016)
Please complete this application and budget and ensure that the budgeted
amounts and Identified Needs align properly. Final approval is contingent
upon these criteria. The State Agency (SA) should complete the items.
 A delinquent facility should submit the completed application along with
the completed signature page (top of page 3) to the SA contact person.
 The SA will review the application. If revisions are required, return the
application to the facility/institution for those revisions.
 If the application is approvable by the SA, the contact person completes the
signature page (bottom of page 3) and submits the application and agency
budget to the Colorado Department of Education by Friday, June 26, 2015.
 Carefully consider the length of time required to meet the Friday, June 26,
2015 deadline as you:
1. Complete the needs assessment
2. Consult with stakeholders concerning the needs identified in the
assessment
3. Meet with the SA to review the application
4. Revise and complete the application
5. Give the SA time to obtain signatures and submit the application and
budget to CDE

Element #1: Identify Students
 Identify the criteria (including the name of the specific assessments) used for selecting
students for participation in the Title I, Part D targeted assistance program.
To address this point, you must clearly describe the criteria and/or process for identifying
students including a list of the specific reading and/or math assessments that are used.
Element #2: Educational Program
 Describe the additional educational assistance that provided to students identified as in
need of help to meet standards in reading and/or math.
To address this point, you must describe the program and services funded by Title ID and
include a discussion of when those services are provided and by whom.

List the scientifically based research instructional strategies and materials that will be
funded by Title ID to address identified needs of students. Explain how instructional
strategies and materials are chosen.
To address this point, you must clearly define the needs of students and list the researchbased instructional strategies and materials that will be used to address those needs.


Element #3: Evaluation Plan
 Describe the process that is in place for monitoring the effectiveness of the program as
a whole as well as the progress of individual students in the program.
 Identify the steps taken when students are not making progress.
To address these points, you must explain how the facility will assess the effectiveness of
the Title ID program and each student receiving service through the Title ID funds.
Element #4: Professional Development (PD)
 Describe the high-quality professional development activities that implemented in
support of research-based strategies.
 Identify the specific student needs that will be addressed by the PD

quality and are tied to the needs identified above in Element #2.


 Explain how these activities coordinate with and are integrated into programs
developed to address identifies student needs.

Element #6: Transition
 Describe in detail how the transition needs of students at the facility are identified.

the described activities listed in the text. The amount of funding to be used for transition
must be 15% to 30% of the total yearly allocation.

2014-15 year must be submitted with the new application.
Requests for new equipment must be submitted on this form accompanied by a
narrative justification in Element #2.
