On behalf of the Department of <DEPARTMENT>, we are pleased to offer you an
appointment as a(n) <RANK> of <DEPARTMENT>. This is a non-tenure track, 9-month
appointment for the <ACADEMIC YEAR> academic year with an academic year salary

to have you become a faculty member at Utah State University (USU). Please note that
this offer is contingent upon a satisfactory background check.
Your appointment at Utah State University officially begins <August 1, 2016>, for the
purpose of benefits coverage. We would like you to be at Utah State University no later
than <August 15, 2016>, and your appointment will continue through the <2016-2017>
academic year. Renewal of employment will occur annually, assuming acceptable
performance of role assignments. Your role will be <XX% Teaching>, and <XX%
Service>. Your role assignments, in the form of a role statement and an annual work plan,
will be reviewed annually with the department head. Please read the attached role
statement very carefully and return the signed copy along with this letter of offer.
For purposes of annual evaluation, your efforts and accomplishments will be assessed in
the areas of <teaching, research, and service> to the department, college, university,
profession, and community. Your primary area of emphasis will be
<teaching/research/service>, and you will be expected to achieve and demonstrate
excellence in that area. You will be expected to be effective in the other <one/two> area(s)
of responsibility.
Your classroom teaching role will be assigned annually by the department head in
consultation with the dean and will be based on department needs as well as your
expertise. Utah State University is committed to providing an outstanding educational
experience to its undergraduate and graduate students, both inside and outside the
classroom. Accordingly, the department, college, and university expect you to establish
and document a record of effective teaching and mentoring, and sustain that record over
time.
You will have access to Utah State's retirement, medical, dental, life insurance, and long
and short-term disability programs with a current value of approximately 45% of your

USU will cover moving ordinary and customary personal and household goods attendant to
your relocating to Utah up to <$x,xxx.xx>. A copy of the university policy governing such
matters is available at our website (http://www.usu.edu/hr/files/uploads/Policies/335.pdf).
In order for the University to make direct payment for moving your personal and
household goods, you must use the moving company designated by the university.
If applicable: In addition, we will cover <up to $x,xxx.xx OR the full costs> for a trip to
Utah for you and your spouse to secure appropriate housing and will cover <up to
$x,xxx.xx OR the full costs> for the final trip for you and your family to relocate to Utah.
For your information, per Internal Revenue Service (IRS) regulations, these payments (not
those in Item #2 above) must be reported as income and are subject to payroll taxes. On
your annual income tax return, you will be able to claim deductible moving expenses. You
may want to consult a tax accountant on these matters or refer to IRS Publication 521,
Moving Expenses, for detailed information regarding deductible moving expenses.
