The work term evaluation is conducted at the mid-point and at the end of each work term. The form that follows is used at both the midpoint and the conclusion of the work term to provide continuity for the student and the supervisor.
Typically, a Coordinator from the University of Toronto’s Arts & Science Co-op Office visits the student and the supervisor after the midpoint of the work term to review the results of the evaluation. The mid-term visit is comprised of three parts:



20-minute visit one-on-one with the student to discuss student’s experience to date and review accomplishments;
20-minute visit one-on-one with the supervisor to discuss student’s roles and goals and review performance to date;
20-minute visit with both the student and the supervisor to confirm performance strengths and learning needs, identify any
additional training or mentoring required and respond to student or supervisor questions.
At the end of the student’s work term, the Supervisor meets again with the student to conclude the evaluation, reviewing areas of
strengths and recommendations for further development and discussing subsequent employment. At the end of this meeting, the
Supervisor should make two copies of the completed form, one for the student and one for the organization’s records. The original is
required for the University to credit the student with successful completion of the work term, and therefore must be returned to the Arts
& Science Co-op office by mail at the above address or by fax at (416) 287-7273.
