Directions: Throughout the semester, meet with and observe the candidate for evidence of attaining standards. Rate the candidate’s knowledge, skills and dispositions according
to the scoring below. The assessment should be discussed with and signed by all parties and given to the university supervisor during the candidate’s final assessment and returned
to the Teaching Performance Center. This assessment includes a numerical rating of competency in the area of intended certification as evidenced during the internship.
Performance in each area is rated as:
1 = Does not yet meet the expectations for the standard.
2 = Meets the expectations for the standard.
3 = Exceeds the expectations for the standard.
N = Not applicable for this evaluation (The specific NCSS theme was not part of this lesson.)
