Faculty Credential Inventory – Form 1 – Instructions
The Faculty Credential Inventory (FCI) is to be completed by the Department Head
1. Date: Enter date the Faculty Inventory Form is completed.
2. Name: Enter the Faculty name.
3. CWID: Enter the college-wide ID.
4. College: Check the appropriate college for which the faculty member has been assigned.
For joint appointments, select all colleges and apply.
5. Department: Enter the assigned department. For joint appointments, select all colleges and
apply.
6. Primary Teaching Discipline: Enter the discipline area a faculty member has been
assigned to teach at the time the form is completed. Note: A new FCI must be completed
anytime the teaching assignment changes.
7. Degree Earned Information: Enter all information for each degree or professional
certificate earned.
8. Qualification Measure: Provide qualification information for discipline areas or specific
courses a faculty member has taught or been assigned to teach at the time the form is
completed. Note: A new FCI must be completed anytime the teaching assignment changes.

List a discipline prefix (CHEM, PSY, MUS,
etc.) if the faculty member is:

Qualified to teach every course within the
discipline. Include only one prefix per line.
Note: If a faculty teaches both UG and Grad
courses under the same discipline prefix, the
prefix will have to be listed twice.

specific courses under a discipline, list the
specific course (BA 301, SWK 225, etc.).
Include only one course per line.
Course Level: Select either UG or Grad for each
discipline prefix and/or course listed in the first
column. Only select one for each line. Note: If

under the same discipline prefix, the prefix will
be listed twice on separate lines.

13. Graduate Faculty Status: Indicate the graduate faculty status at the time the form is
completed. This form is not an application for graduate faculty membership. It is intended
to reflect an existing status. Graduate faculty membership requires a formal application and
approval by the Graduate Council. Note: A new FCI must be completed anytime the
graduate faculty status changes.
14 - 16. The following table is to be completed only if a faculty member does not hold a
terminal degree in the teaching discipline, which is indicated under the Qualification section
of the table on page 1.
An entry is required for each discipline prefix and/or course that is qualified with a degree
outside the teaching field plus other qualifications.
For undergraduate courses, Tables 14 & 16 must be completed.

credentials, including the official transcripts, indicating the faculty meets SACS
credentialing guidelines. The signature from the Dean of Graduate Studies is required to
verify the graduate faculty membership/status.

Faculty Credential Inventory Supplemental Qualifications Form 2- Instructions
The supplemental qualification is to be completed only if the faculty member lacks a terminal
degree in a teaching discipline.
1. Date: Enter date the Faculty Inventory Form is completed.
2. Name: Enter the Faculty name and college-wide ID.
3. CWID: Enter the college-wide ID.
4. College: Check the appropriate college for which the faculty member has been assigned.
For joint appointments, select all colleges and apply.
5. Departments: For joint appointments, enter all departments that apply.
6. Rank: Check box to indicate rank.
7. Series: Check appropriate box to indicate series.
8. Education – Graduate Degrees: Indicate degrees held, giving major and institution that
awarded the degree. (Official transcripts for all graduate degrees are required.)
9. List Graduate Hours in the Teaching Discipline for Courses listed in Table 14/15 on
the FCI. (official transcripts required)
10. List Graduate Hours Related to the Teaching Discipline for Courses listed in Table
14/15 on the FCI. (official transcripts required)
11. Exceptional Scholarly or Creative Activities: Provide information for any exceptional
scholarly or creative activities. Documentation is required.
