Note: A new FCI must be completed anytime the teaching assignment changes. 7. Degree Earned Information: Enter all information for each degree or professional certificate earned. 8. Qualification Measure: Provide qualification information for discipline areas or specific courses a faculty member has taught or been assigned to teach at the time the form is completed. Note: A new FCI must be completed anytime the teaching assignment changes. • List a discipline prefix (CHEM, PSY, MUS, etc.) if the faculty member is: • Qualified to teach every course within the discipline. Include only one prefix per line. Note: If a faculty teaches both UG and Grad courses under the same discipline prefix, the prefix will have to be listed twice. • • If a faculty member is only qualified to teach specific courses under a discipline, list the specific course (BA 301, SWK 225, etc.).
