COUN 552 –Internship I or II Application

*School Counseling -- No Internship is offered unless you have a site at a year round school site, which will need Dr. Linda Ball’s approval.

for information on the Student Affairs internship for the Fall and

Spring semesters ONLY.

Prerequisites:

Internship I students must have completed Practicum (COUN 551) satisfactory, with “S" grade.

Internship II students must have completed Internship I (COUN 552) satisfactory “S" grade.

Applications submitted by April 8, 2016 will receive first consideration for available classes. Late applications are not guaranteed consideration or may need to accept an alternative location or time for your class .

You must complete the online Internship Orientation before April 15, 2016 (Click on "Internship Orientation" on the

"Clinical Applications" page). You must provide complete field site information to the Candace Chuyou-Campbell before

April 29, 2016 . Please follow the guidelines established in the Checklist for Internship for Summer 2016. (Click on "Clinical

Forms" in the "Clinical Applications"). Failure to meet deadlines may result in your being dropped from the course.

PLEASE DO NOT WAIT UNTIL THE LAST DAY TO SEND IN YOUR APPLICATION. WE NEED TO HEAR

FROM ALL STUDENTS WHO WANT TO TAKE THIS CLASS AS SOON AS POSSIBLE.

You will be notified of your acceptance to your section via Leo email.

Application must be completed in its entirety to be considered (more than 1 page)

IMPORTANT School Counseling Students--If you are following the School Counseling Track, you must comply with Professional Certification program, also. Make sure you have submitted an application and turned in all the required documents to their office (this includes a copy of your service record). This requirement must be completed before you can be considered for COUN 551.

Complete personal counseling requirement (Minimum of 6 sessions of individual counseling). Can be met at no charge through sessions in Community Counseling & Psychology Clinic (903)886-5660

(Commerce), Harold Murphy Clinic (469)302-6318 (McKinney), or through a counselor of student’s choice (fees may apply and are the sole responsibility of the student). Student submits documentation of participation for placement in file (Masters Handbook Spring 2015).

Which campus do you prefer? (Indicate 1 st , 2 nd , or 3 rd , but ONLY if they are good options for you—Do not indicate a location unless it is a good option for you. If you will not be able to attend classes on that campus, then do not indicate it as a good option.)
