Teacher Education Candidate
Recommendation Form
Critical Dispositions: In addition to standards that guide the preparation and licensure of candidates’ knowledge of content and ability to teach,
candidates are also evaluated on critical dispositions essential for success in education. Critical Dispositions are defined by NCATE as “attitudes,
beliefs, values, and commitments that influence behaviors.” Ten of these are vital themes in the Otterbein University Teacher education program.
We expect these dispositions to be evidenced in the field as candidates practice their professional skills.
For Teacher Education Program Candidates: A positive recommendation from a member of the Otterbein faculty or staff who can assess the
student’s critical dispositions is required for admission to the Teacher Education program.
To the Applicant: Under provision of the Family Education Rights and Privacy Act of 1974, you have the right to review your educational records
after you are enrolled at Otterbein University. The Act further provides that you may waive your right to see recommendations for admission.
Please indicate below whether or not you wish to waive this right by circling the appropriate phrase and signing your name. Not signing will be
interepreted to mean that you do not wish to waive your right of access. I choose an item. Any right of access that I may have to this
recommendation form.
To the Otterbein Faculty or Staff solicited to complete this form for a Teacher Education Candidate: The individual whose name appears below is
seeking admission to our teacher education program at Otterbein University in order to complete teacher licensure. Please be as complete and
candid as possible about the individual. Return this form to the Academic Department Assistant (Claire Parson cparson@otterbein.edu) in the
