ENVS VEHICLE REQUEST FORM
Please complete the following information for your trip(s) and return to Fabina B. Colon (fcolon@ithaca.edu),
1 week prior to trip, in order to guarantee vehicle availability and complete reservation. Requests will be
entered into the internal ENVS budget tracker to keep current with charges (Facilities often bills 1-2 months
after trip occurs!).
Note: There is a $10 per vehicle no-pick-up fee if Facilities gets less than 48 hours notice of trip cancellation.
Also note: Ithaca Dispatch is a fall-back option if IC vehicles are not available or trip constraints preclude selfdriving (e.g., different drop-off and pick-up locations)
