SECTION I - 4-H ACCOMPLISHMENTS
A.
Size and Growth in Projects
What have you done in your projects? This should be broken down by each project to show growth and progress.
Information you might include: number of recipes tried, number of experiments completed, number of animals raised,
monetary costs involved, money made, number of books or articles read related to project, number of exhibits shown.
B.
Knowledge and Skills
List four or more things you have learned for each project taken. This could be both positive and negative learning
experiences. For example: learned the specific feeding needs of my animal as it grows; why to careful with hot dishes;
why my clay pot blew up in the kiln; learned about a specific disease or problem, etc. Group by project area.

Participation in Activities

List 4-H activities you participated in. Examples: Attended visual arts workshop, livestock judging contest, tours, classes,
etc. Use titles and descriptions of the events. Group by project area.

Leadership Experiences
How have you excelled as a 4-H member? List your leadership experiences in 4-H only. This could include committees
served on or chaired, offices held, taught members a craft; led an activity for club, talks or demonstrations given, helped at
food stand, made a radio spot, submitted a show book cover design, etc. Group by project area.

Participation/Leadership in Activities Other than 4-H
List activities you participated in for school, community, church, etc. on your own or with other groups. What have you
done to help others? Also state if you held any leadership positions with the groups you belonged to. Group by project
area. Examples: Council of Churches--donated for their holiday food baskets; Nursing Home--sang songs or made cards
for residents; Basketball team--manager; School library--helper.
