name and the level, if applicable, for the project record(s) to be judged.

(Please check boxes to verify all required documents are included in your records binder)




STEP 1: Include your name and 4-H Club on the outside of the notebook or binder.
STEP 2: Include this form as the first page of your notebook.
STEP 3: Complete and date the “My 4-H Activities” record; include it as the second page.
STEP 4: Complete a “Project Plan” record sheet according to your age group for each project in which
you have enrolled and exhibited in this year.
*Record sheets may be submitted for categories in which you have enrolled but did not exhibit.
o Multiple projects may be grouped by project area and submitted on a single record form (e.g., a
member with a Clay project and a Fiber project may fill out one Visual Arts record).
o Projects in which the member exhibited both as an individual and as part of a group may be
recorded on a single form.
o Each project in the curriculum area should have its own Project Plan record (e.g., Visual Arts and
Public Presentations must have separate records). Exception: Foods projects may be
recorded on the same form (a member who exhibited in Food Preservation and Sports Nutrition
may fill out one Foods record).
o County projects should not be grouped with State Curriculum projects (e.g., separate record
forms should be submitted for My Favorite Family Recipe, etc.).
 STEP 5: Circle the project level participated in for all records that you wish to be judged on the
Lake County 4-H Projects by Curriculum Area form (reverse page). If the project area does not have
levels (i.e., Visual Arts), simply circle the name of the project.
 STEP 6: Please date each record with “2009-10” in the top right corner. Each record must also be
signed.
 STEP 7: Record book must be index tabbed and labeled according to the following curriculum areas:
