Child Development Training Consortium Application Forms NEW PROCESS – MUST BE DONE ONLINE! Instructions are attached The Child Development Training Consortium has been established to improve the quality of programs for children by helping students pay for their college course work while they are employed in Child Development programs. Through the reimbursement of College fees, the Consortium will help students meet the requirements of the California Child Development Permit. For those of you who would like to participate in the Child Development Training Consortium and receive reimbursement (whole or in part) for your College fees, please READ THE ATTACHED INSTRUCTIONS FOR ONLINE APPLICATION, and return the printouts and required receipts to your Instructor or Janice Silver on or before October 9, 2015. For assistance and further information, please see one of our tutors in the Child Development Student Resource Center – Room 200. In order to qualify: 1. You must be currently employed in a child development program such as: public or private Child Development Center, Head Start, Early Education Center, State Preschool, SRLDP, TK, School Age Care program, Infant Center, Family Child Care Home, LAUSD kindergarten classroom only. 2. You must fill out the online participant profiles completely. Every question must be answered in order to be accepted. Include all classes in which you are enrolled. The last five digits of your social security number as well as your student ID number are required. 3. Your director, principal, or employer must fill out Section C of the application. The supervisor must sign the application. Family Child Care providers may fill this out themselves and must include their license number. You will be able to download this page AFTER you submit the online form to the coordinator (Janice Silver) 4. COPIES OF YOUR REGISTRATION PLUS RECEIPTS FOR TEXTBOOKS PURCHASED MUST BE STAPLED TO THE BACK OF THE PRINTED PAGES. If budget permits, partial reimbursement of textbook fees will be given. 5. After the applications are received, the Consortium will prioritize candidates and determine the reimbursement amount. Students will receive reimbursement by check through the LAMC Business Office.
