 the standard affirmative action statistical form (includes a check list)

 the search plan including o position description o qualifications o description of the search procedure o description of the committee’s efforts to attract a diverse pool of candidates

 a report from the search committee including o a list of all the under represented minority and women applicants, annotated to show the disposition of each application o a list of all the individuals interviewed o a description of the interview and decision process o a review of the finalists and key factors in the final decision o the CV of the successful candidate o the CVs of all under represented minority applicants who are U.S. citizens or permanent residents, including a statement about why the applicant was not interviewed or selected.
