Notetaking Policy Students interested in receiving notetaking services as an accommodation to mitigate the effects their disability has on their ability to obtain lecture notes, must first provide documentation of their disability and need for requested services to the Office of Disability Services (ODS) . The Associate Director for Disability Services will review the student's documentation and in consultation with the student, determine whether notetaking services would be an appropriate accommodation. (Other service options may be determined to be more effective or appropriate depending upon students documented needs.) If you are eligible to receive notetaking service, ODS will recruit student notetakers from your classes to provide copies of their personal notes Notetaking Process: 1. Register with the Office of Disability Services by providing appropriate documentation of your disability; 2. Meet with an ODS representative each semester you would like to receive notetaking services; 3. Attend all of your courses to determine if notes and/or PowerPoint slides will be available to the class (please note, if comprehensive notes are available prior to each lecture, notetaking services may not be provided); 4. Complete a Notetaker Request form for those classes in which supplemental notes are not provided; 5. Once a notetaker has been secured by ODS, a folder will be created where you will retrieve your notes; 6. You will retrieve your notes on a weekly basis and sign-out that you have picked-up your notes; 7. If notes are not retrieved by the student and/or the student does not respond to the notice/s within a week, s/he will need to meet with the Associate Director to determine whether to continue the service for the remainder of the semester; 8. If notes are not satisfactory, the student must notify the department by completing a Note Receiver’s Form which can be found in ODS; 9. Notify ODS if you drop a class or no longer need notes for a class for which a notetaker has been hired. Please note: This service does not diminish student recipient’s responsibility of taking his or her own notes. We cannot guarantee notes will be provided if request is made within the last 3 weeks of the semester. Every effort will be made to hire a notetaker immediately after the request is submitted; we will ask the notetaker to provide back notes if necessary. I have read and understand the Notetaking Policy outlined above and agree to comply. _____________________________________________________________________________________ Student Signature / Date _____________________________________________________________________________________ Printed Name / MUID _____________________________________________________________________________________ Office of Disability Services Representative / Date Office of Disability Service Marquette University P.O.
