The “Top 4-H Recreation Chairman/Committee of the Year Award” will be presented at
4-H Achievement Night. Nomination forms must be completed by a club leader. The
Recreation Chairman/Committee must demonstrate:
 How they have contributed to the 4-H Club;
 Their ability to lead while helping other club members to develop as youth
leaders by encouraging the “youth-run” philosophy;
 That they have motivated members to participate;
 That they involved all members in recreation activities and helped to build club
cohesiveness.
NOTE: Both the Recreation Chairman/Committee must be nominated.
Please complete the following by summarizing the Recreation Chairman’s/Committee’s
achievements/participation in each of the following areas. Use examples when possible.
