CHS Graduation Ticket Declaration Form
Each graduating senior is allotted up to nine (9) tickets for seating at the ceremony on Saturday
May 28, 2016 @ 8 a.m. In an effort to accommodate the needs of each family at this year’s
graduation ceremony, please read the following statements and complete the form below.




You have the option to decline any or all of the nine (9) tickets allotted to your
graduating senior(s).
If you decline any or all of the nine (9) ticket allotment, you will not be able to make a
request for additional tickets at a later time.
This form is the only way to make a request for additional tickets above the nine (9)
ticket allotment.
A lottery will be conducted for additional tickets. A request for additional tickets is not
guaranteed and/or may be subject to a two-ticket cap if your request is selected in lottery.
This form must be returned to the CHS administrative office no later than Friday, March
4th, at 4 p.m.
For those requesting additional tickets:
I am requesting
tickets in addition to the nine (9) tickets allotted per senior.
For those not requesting more than nine tickets:
I do not need all or any of the nine (9) tickets and would like to return
tickets allotted per senior for other families in need.
