bottom of this page. Your signatures will indicate your understanding of the requirements
of the successful completion of the state mandated health course.
To be successful in Health this Year:
A. You will need to bring the below listed materials to class each day:
1. A binder - 4 sections
2. A pen and pencil
3. Your issued textbook
B. You will be responsible for:
1. Attending class on assigned days (cutting is an unexcused absence).
You will be accountable for missed work.
2. Bringing to class the required materials, i.e., notebook.
3. Completing all homework assignments and submitting them on requested dates.
4. Being prepared for the taking of quizzes and tests, relating to classwork and text
information.
5. Completing the health project(s) agreed upon by my teacher and myself.
6. Seeking teacher help when you do not understand the material assigned.
My report card grade will reflect the results of:
A. Tests: 2-4 per marking period (Weight of One (1) grade per each unit test).
B. Project(s): 1 per marking period (Weight of One (1) grade).
C. Notebook check: 1 per marking period (Weight of One (1) grade)
D. ****Homework Assignments (Weight of One (1) grade/Participation).

****Each student receives 100 as a homework grade. In order to keep the 100, they
must hand in the assigned homework on the day it is due (unless they are absent).
Failure to do so will result in points deducted from your homework grade.
