Research Degrees Supervisor CV form What this form is for Who should complete it How it should be submitted To provide details of a proposed supervisor’s academic history and experience, to support the case for appointment as a supervisor to a named research student. It replaces the SCV5 and ARC4 forms previously in use. Any person being proposed to supervise an Open University research student. This form should be submitted to the Research Degrees Office by email by the applicant’s Deanery, together with the Applicant recommendation form, in the case of directly-registered students, or the Application for registration for a research degree, if the student is registered in an Affiliated Research Centre. It should also be used to support any notice of changes to supervision arrangements for a student. Right to work in the UK – directly registered programme only The Open University is required by law to ensure that anyone appointed to undertake work on its behalf has the right to work in the UK. You are asked to provide a copy (scanned copy or photocopy) of your passport or other form of documentation with this form. Before confirming your appointment, we ask that you also visit the Research Degrees office in Milton Keynes before your first supervision meeting to show us your original documents for checking. Alternatively, you can present your documents at any of the Open University’s regional or national centres before the date of your first supervision meeting (an appointment is normally required).
