Senior High Mathletes Permission Slip

to attend all 6 Senior High Mathletes competitions for the
2015-2016 season. The dates for those competitions are:
Tuesday, October 20, 2015 @ Carey HS
Tuesday, November 17, 2015 @ Sewanhaka HS
Tuesday, December 1, 2015 @ Elmont HS
Tuesday, January 5, 2016 @ Floral Park HS
Tuesday, February 23, 2016 @ NHP
Tuesday, March 1, 2016 @ Carey HS
I understand that my child will travel by bus to another
high school in the district and that students may be
returning to New Hyde Park High School after 6:00pm on
any contest date depending upon the length of the
competition.
