COVER Oregon Logic Models Faculty Template for SOARS
Faculty Logic Model COVER Oregon Narrative Summary
Develop your Plan of Work in a Word document and paste into the SOARS online system.
This allows you to be more thoughtful with your planning and develop a higher quality plan
that will be easier to report accomplishments toward.
Each Logic Model asks for the Program Title and the Work Area:

Below is sample text for Healthy Families and Homes questions under the Long-Term
Outcome/Impact section. Add, delete, and edit text to reflect Cover Oregon programming in
your unit. Copy and paste into SOARS.
Describe what will be done and who will be reached in narrative form.
Describe the staff make-up of your unit that will carry out the 2014 COVER Oregon Education
Plan in the beginning of your narrative in this section.
The overall goal is to increase awareness and understanding of COVER Oregon
utilizing the educational network of the Oregon State University Extension Service.
To provide Oregonians with the educational materials that support increased
knowledge of COVER Oregon.
Education Delivery Planned: (Copy as many of these sentences as you need to complete a
summarized picture of the type and amount of education you plan to deliver for FFY2014 in

Briefly describe the process of how the need for this program was determined (needs
assessment).
Because Extension faculty and staff live and work in the communities they serve, they
have deep knowledge and appreciation for the characteristics of the region. To best
serve each county, we will utilize the long-term relationships with local partners
already established. We will add outreach and education related to COVER Oregon
to our existing programs, where possible, as we already reach many of the “hard to
reach” audiences that COVER Oregon targets.
Locally, in
County, needs will be determined while meeting with decision makers
and community leaders (community collaboration partners) reaching an estimated
individuals resulting in
direct contacts to promote COVER Oregon.
As a result of these meetings, sites were determined and plans made to set up events
and distribution of COVER Oregon brochures and materials.
Sample: Locally, in Jackson County, needs will be determined while meeting with decision
makers and community leaders (community collaboration partners) reaching an estimated 10
individuals resulting in 20 direct contacts to promote COVER Oregon.
What are the short-term (learning) outcomes for this program? (Narrative or outline form)
Families will receive educational materials and learn about COVER Oregon and what
they need to do to access health insurance.
What are the medium-term (action, behavior change) outcomes for this program?

Families will access the COVER Oregon website and make contact with application
assisters in their community.
