INSTRUCTION SHEET FOR CERTIFICATE OF ACCOMPLISHMENT A Certificate of Accomplishment certifies a competency in a given area and can require as little as 1 unit or as many as 11.99 units. The total required units MUST BE FEWER THAN 18 UNITS. Certificates of Accomplishment are not posted on the student’s transcripts. The standards approved by the AD/GE subcommittee and the Curriculum Committee must meet the following requirements: 1) 50% or more of the unit requirements for this certificate must be completed in residence; 2) a minimum grade point average of “2.0” for the certificate courses, OR a minimum grade of “C” in each course OR both. Your department can set higher limits if so desired. All certificates of accomplishment must be tied to a LBCC curriculum guide. If your department would like to offer a Certificate of Accomplishment, the following approval process must be followed. STEP 1. TASK Meet with the AD/GE Representative or Articulation Officer. Complete the one-page form “Curriculum Report Form for Certificate of Accomplishment” This form must be electronically word processed. A hard copy must be signed by department head, school dean, and the AD/GE Representative or articulation officer. The signed original must be a “clean copy” with no date stamps or any other markings on the front, as it will be presented to the Board of Trustees. 2. 3. Submit the signed hard copy “Curriculum Report Form for Certificate of Accomplishment” to the AD/GE Representative or Articulation Office LAC, Room N102, Mail Stop #R7 AND also e-mail the electronic version of the completed form to the AD/GE Representative or the Articulation Officer. Once your request is submitted to the AD/GE Representative or Articulation Office, it will be placed on the agenda for the next AD/GE subcommittee meeting. You will be notified. A representative from the department needs to attend this meeting to address any issues or questions that may arise. If request is approved, it will be forwarded to the Curriculum Committee for approval. If that committee approves the request, the request is then forwarded to the Board of Trustees for final approval. At that point a copy will be forwarded to the chair of AD/GE. Once the Board of Trustees approves the request, the certificate can be advertised (put on curriculum guides, department website, OSCA website, flyers, etc.). Generally, department attendance is not required for Curriculum Committee or Board of Trustee meetings. NOTES: The Curriculum Report Form for Certificate of Accomplishment cannot exceed one page. The Curriculum Report Form for Certificate of Accomplishment must be submitted to the AD/GE Representative or the Articulation Office electronically and as a hard copy to include signatures. The AD/GE Representative or the Articulation Office will notify the department after the necessary approvals have been obtained. Certificates of Accomplishment for 12 units or more can be forwarded to the State for approval to become a Certificate of Achievement, contact Academic Services to initiate this process after Board Approval of your certificate.
