Record of Change – Quarterly Report Documents
Changes have been made to three quarterly report documents. It was found that the three
documents had the incorrect P.O. Box listed for DEP, where the forms are submitted.
The correct P.O. Box is 8555.
The three forms that were changed include:
1. Attachment F – Level 2 Plan Approval Data
2. Attachment G – Manure Management Reporting
3. Attachment H – Nutrient Management Program, Level 2 Quarterly report
Conservation Districts should start to utilize the new reporting documents for quarterly
reporting for the 4th quarter of 2015 (October – December).
The three new quarterly reports are attached and can also be downloaded from the
Nutrient Management Website at http://extension.psu.edu/plants/nutrientmanagement/scc/manual/chapter-6
Please file this notice and the attached three forms in Chapter 7, Record of Change, in
version 3.0 of the Pennsylvania Nutrient Management and Manure Management Manual
Program Administrative Manual.
Additionally, in Chapter 6 of the manual, place a red X over the three outdated forms and
place a note on the crossed out forms, to refer to Chapter 7, record of changes.
