Community Card Application 2015
Thank you for your interest to obtain a CWRU Community Card! As a card holder, you will have
access to the university’s campus facilities, services and notification about signature special
events. Some of the facilities include:








Village at 115 Track and Field
Thinkbox Entrepreneur Center
Kelvin Smith Library
Tennis courts
Basketball courts
Alumni House
One to One Fitness Center (annual membership required – discount provided to card holders)
Adelbert Recreation Center and Gymnasium (annual membership required)
Please complete the information below and once completed, take application to CWRU’s
Division of Access Services. It is on the lower level of CWRU’s Crawford Hall, located at the
intersection of Euclid Avenue & East Boulevard (west of Amasa Stone Church or across from
Severance Hall). In addition to application, please bring a government issued photo I.D. (state
I.D., driver’s license, military I.D.) and your application fee of $25. Please be advised your picture
will be taken for the Case Community Card. For directions, please call (216) 368-2273. 15
minute parking is available in the Campus Visitor’s Parking Lot located adjacent to Crawford
Hall.
