Consult the host institution’s website and/or course catalog and make a list of the courses you would like to take while studying
abroad or participating in the National Student Exchange. Be sure to include several alternative courses so that you have flexibility in adding or dropping courses. With your selections, complete the left side of the Course Selection & Equivalency Table located at the end of this form.
NOTE: If you plan to study away as part of a year-long program and the host institution has not posted coursework for the second semester, do not panic. Bring a blank copy of this form along with you overseas and complete the table when the information becomes available. Once you have obtained the necessary course information, you can fax or scan/email the form to your academic advisors.
Take the Course Selection Table to your Academic Advisors
Be sure to bring copies of course descriptions from the host institution website or catalog along with you when you meet with your academic advisors. Discuss how the courses taken away will fit into your academic plan. You may need to meet with general education advisors as well as advisors in your Major or Minor program of study. If courses will transfer as elective credit, your advisor must determine whether the course will transfer as upper-level or lower-level coursework.
Note: If you plan to graduate immediately following your study abroad experience, be sure to apply for graduation as early as possible and compare the Registrar’s audit with your completed advising agreement to ensure courses completed away will fulfill all outstanding requirements. Transcripts must be received from the host institution within 42 days of UWGB’s conferral date or your graduation will be moved to the next available term.
Request applicable signatures from your academic advisors.
Submit the form to the Student Service Center (SS 1100), University of Wisconsin-Green Bay, 2420 Nicolet Drive, Green Bay, WI 54311.
Converting credits into UW-Green Bay semester hours accurately is essential in ensuring you continue to make progress toward your degree while studying abroad or participating in the National Student Exchange. Use the tables below when planning your academic experience. Registration in excess of 18 UW Green Bay credits will incur additional tuition and fee charges for each credit above the tuition plateau. *Hessen ISU/IWU programs are 6 credits total (two 20-hour seminars and one 40-hour language course; credit points are not offered for workshops).

To the Academic Advisor:
The following proposal lists the courses the student plans to complete during his/her study abroad program. Please consult with the student regarding the appropriateness of courses, prerequisites needed, and progress toward degree completion. Signature of this form authorizes the transfer equivalency. If no UWGB equivalent exists, indicate whether discipline elective credit is upper- or lower-level. Please note that courses taken at institutions utilizing a quarter schedule receive 0.67 UWGB credit for each credit completed. Students studying at a
European institution employing ECTS credits receive 0.5 UWGB credits for each ECTS credit completed. Further, it may be necessary to group 2 or more courses together at the host institution to satisfy a 3 or more credit course requirement at UWGB. Please contact the
Registrar Office x2657 or the Office of International Education at ext. 2190 with any questions.
