SPECIAL NOTE: Addendum #4 information shall supersede the information contained in
the Contract Documents
This Addendum forms a part of the Contract Documents and modifies the original Bidding
Documents dated January 20, 2014 and all addenda and clarifications. Acknowledge receipt of
this Addendum in the space provided on the Bid Form. Failure to do so may subject the Bidder to
disqualification.
The contractor is responsible for the removal of all existing fire alarm devices and related
conduit/wire mold.
ISG fire alarm layouts attached for Fire Lite system. Contractor is responsible to coordinate
device counts and provide this information in his proposal submission.
The following devices shall be included in your proposal for the Fire Lite fire alarm system in
addition to the quantities quoted:





o Add a CO detector in the basement
o Add a heat detector in the basement
o Add a heat detector in the attic
346 Washington Street
o FACP to be located on west wall
o Add a smoke detector in the entry vestibule
o Add a heat detector in the attic
356 Washington Street
o Add horn strobe in bedroom 101T for ADA compliance
o Add horn strobe in bedroom 213
o Install magnetic hold open on door to kitchen at west entrance
o Install magnetic hold open on door to laundry room from common room (layout
reflects a wall in the north east corner of room, door is present)

o Add heat detector in laundry roof off kitchen
o Install carbon monoxide detector in front entry corridor
