This Addendum forms a part of the Contract Documents and modifies the original Bidding
Documents dated December 20, 2014. Acknowledge receipt of this Addendum in the space provided
on the Bid Form. Failure to do so may subject the Bidder to disqualification.

The prebid meeting addressed the following items:
1. Schedule: Completion dates shall be strictly adhered to.
1. Coordination of the work is critical.
2. Safety: Zero accidents. All contractors shall practice safe workmanship practices
3. Surrounding areas/buildings will be occupied throughout the project schedule.
4. OSHA Training: Contractor shall provide minimum 10 hour OSHA Training Certification
for workman and minimum 30 hour OSHA Training Certification for Working Foreperson or
Supervisor to Owner prior to start of work. Reference 01015 General Requirements.
5. Hazardous Materials: Contractor shall coordinate all required ACM removal with Owner.
Inspection reports are available in the Owner’s office and will be posted to the MM FY15
website.
6. Lead Materials: All painted surfaces are to be treated as positive for lead containing paint due
to the construction of the building prior to 1978. All contractor personnel working on the
project shall be certified by the U.S. Environmental Protection Agency (EPA) for lead based
paint activities.
7. Abatement costs will be borne by the Owner. Contractor to coordinate utility disconnections,
fixture removals, and demolition prior to abatement removal with the Owner.
8. Parking; Parking on grass surfaces is strictly prohibited. Parking in driveway of the
woodframe is acceptable. Off street parking is available. Additional parking is available at
the Vine Street parking lot.
9. It is the contractors’ option to a flash drive or CD to obtain available AutoCAD drawings.

10. Contractors are encouraged to check the major maintenance website frequently for new and
updated information. Addenda, specifications and additional project work will be posted to
the website as it becomes available.
11. If out of office message is received when contacting R. Sillasen by phone or email, contractor

12. A bond is not required. Builder’s Risk insurance is not required.
13. Insurance Certificate must name Wesleyan University as additional insured.
Location Plan provided at pre-bid conference.
Existing Fire Alarm shutdowns to be coordinated with Wesleyan University.
All furniture is to be protected throughout the work and cleaned at the conclusion of the project. It is
the contractor’s choice to procure separate storage container for furniture storage.
Clarification Note: Where it is noted in the scope of work to replace stair treads, reference is to
replacement of vinyl stair treads.
Attic and basement windows shall be replaced. Notify Owner of property location where conflicts
may exist.
In all locations where ceiling lighting is identified to be replaced with Incon Lighting delete
reference to Incon and provide ETI Lighting as follows:

The Owner shall hold the contracts of the site contractor, fire alarm contractor and sprinkler
contractor.
General Contractor is responsible to hold all contracts related to the construction scope.
Sprinkler system reduced pressure zone backflow preventer shall be Watts LF909 Series as required
by the City of Middletown. Sprinkler contractor shall provide sprinkler shop drawings with detail
drawing of reduced pressure zone backflow preventer. Contractor shall schedule testing of reduced
pressure zone backflow preventer by City of Middletown Public Works Department.
For all locations, carbon monoxide activation shall report as a trouble and identify carbon monoxide
location.

264 Court Street – Fire Alarm / Fire Sprinkler/Fire Sprinkler Water Supply – Renovation of Third

1. General Contractor to provide cost for Mathews Brothers Noah Webster windows on the

Model # 94014 on interior of window unit. Unit shall be hinged for easy opening.
5. Water service shall be from water main in Court Street.
6. Additional Devices:
o Add a carbon monoxide detector in Bedroom #A2, Apartment A.
o Add a pull/horn/strobe in kitchen of Apartment B.
BID PACKAGE #13A:
18/20 Brainerd Avenue – Fire Alarm / Fire Sprinkler, Vinyl Siding, Window Replacement, Porch
Replacement, Carpentry Repairs, Add Bedrooms
1. Provide costs for Mathews Brothers Spencer Wolcott windows and Harvey Classic windows
on the proposal form.
2. Contractor shall replace basement window with dryer vent. Dryer vent will be terminated in
basement.
3. New doors to newly create bedrooms shall match existing 5-panel door style.
4. Delete reference to remove and infill door in entryway to for new bedroom on first floor at 20
Brainerd Ave Apartment. Door is not present.
5. Water service shall be from water main in Brainerd Avenue. .

1. Are the Window Grilles between the Glass lite cuts to match existing Cottage T –

3. Are the two Attic Stair Windows required to have Tempered Low E with Argon

1. Vermiculite is present in the attic spaces. Contractor to identify any penetrations required
through ceilings below Attic spaces.
2. Water service shall be from water main in Knowles Avenue.
3. General Contractor shall remove window in Bedroom #4 and install new construction
window to match adjacent window style and profile. Provide masonry repair, new lintel and
all related work for a complete installation.
BID PACKAGE #13D:
240 Court Street – Fire Alarm / Fire Sprinkler Project
1. Basement is only accessible through the west elevation bilco door.

floors.
3. Water service shall be from water main in Pearl Street. City of Middletown does not provide
a 4” tap. A 6” tap reduce to 4’ supply is required.

BID PACKAGE #13E:
115 Cross Street – Fire Alarm / Fire Sprinkler, Renovations
1. Water service shall be from water main in Fountain Avenue.
2. This is a partial scope. Complete renovation scope of work by General Contractor will be
uploaded to the MM FY16 website.
Exterior - general
1. Remove entire length of chain link fence and post footings at east side of house.
2. Plug hole in siding and provide siding repairs (see photo sheets).
3. Provide new 2" domestic waterline service to from street line to meter in basement - to
replace existing.
4. Provide new support footings to replace existing at the east side addition. Provide all
required shoring prior to excavating for new footings.
South side porch to second floor
1. Contractor shall demolish south side exterior enclosed porch including stair system, framing,
roofing - salvage existing vinyl siding for reinstallation and repairs.
2. Remove door to kitchen. Frame opening to accept new construction double hung vinyl
window with grid configuration to match existing. Window height to match full size windows
in building.frame knee wall, insulation, sheath, prep and paint. Install interior wall base to
match existing.
3. Install vinyl siding.
North side porch
1. Contractor shall demolish north side exterior porch including deck, framing and railings maintain existing roof.
2. Install new footings, header beam as required, framing, trex composite decking, composite
stair/railing system and composite columns as required to support the existing shed roof.
New composite structural column style shall be 4" square. Maintain existing footprint.
3. Stair / railing system shall be: certainteed panorama composite railing system, balusters shall
be: square.
a. install new vinyl privacy lattice. Provide all framing/anchorage.
b. provide horizontal and vertical azek running boards and stair risers
14. Remove / reinstall gutter and leader to complete the work.
15. Repair siding at railing tie-in locations.
Interior - general
1. Fireseal all thru floor / thru wall penetrations.
2. Remove all existing door hardware throughout the house and replace with new lever handle
hardware and new cores. Coordinate with wesleyan lockshop.
3. Patch, prep and paint the entire 1st, 2nd and attic floors to include all previously painted
surfaces and newly installed paintable materials, walls, trim, 1/4 round, interior/exterior
doors, frames, ceilings, moulding, stair treads, stair risers, stair posts, stair balusters, stair
railings, closets, shelves, exposed conduit, wiremold and piping.
4. Paint shall be: sherwin williams promar 200. Wesleyan white #2532.
a. semi-gloss: kitchen and bathrooms and all previously painted woodwork and

6. Upon completion of all work, the entire house shall be thoroughly cleaned for move-in
condition.
1st floor kitchen
1. Remove existing kitchen entry door, install new kitchen door, lever handle lockset and
peephole. Coordinate hardware core requirements with wesleyan lockshop.
2nd floor kitchen
1. Remove all shelving in kitchen - prep, patch and paint walls.
2. Infill missing false panel at base cabinet face adjacent to dishwasher. Match panel style &
size with existing.
3. Remove dishwasher. Properly terminate plumbing and electrical. Infill opening with new
base cabinet / door - stain & poly to match existing.
4. Clean and polish kitchen cabinets.
2nd floor bathroom
1. Remove commode, vanity, countertop & faucet - salvage all for re-installation.
2. Remove perimeter base trim.
3. Provide luan underlayment for new bathroom sheet vinyl installation.
4. Install new sheet vinyl per specifications. Caulk around perimeter of sheet vinyl installed for
a water tight installation.
5. Install new vinyl cove base trim around perimeter of bathroom.
6. Install transition threshold between bathroom door and adjacent room.
7. Reinstall commode, vanity, integral countertop & faucet.
8. Clean tub / shower. Re-caulk around entire tub / shower unit - maintain shower rod.
2nd floor common room, bedroom #1, #2 & #3
1. Screen existing wood floor and apply 3 coats finish per specification requirements.
2. Patch walls in bedroom #1.
3rd floor hall, bedrooms #4 & #5
1. Screen existing wood floor and apply 3 coats finish per specification requirements.
2. Remove existing wood railings at top of attic stairs.
3. Install new 36" wood railings with balusters per code
4. Patch and repair damaged walls as shown on the photo sheets.

re-installation. Reinstall existing toilet upon completion of the new sheet vinyl floor
installation. Install new supply line and wax ring. Ensure all new and re-installation work
does not leak.
