Schedule: Completion dates shall be strictly adhered to. 1. Coordination of the work is critical. 2. Safety: Zero accidents. All contractors shall practice safe workmanship practices 3. Surrounding areas/buildings will be occupied throughout the project schedule. 4. OSHA Training: Contractor shall provide minimum 10 hour OSHA Training Certification for workman and minimum 30 hour OSHA Training Certification for Working Foreperson or Supervisor to Owner prior to start of work. Reference 01015 General Requirements. 5. Hazardous Materials: Contractor shall coordinate all required ACM removal with Owner. Inspection reports are available in the Owner’s office and will be posted to the MM FY15 website. 6. Lead Materials: All painted surfaces are to be treated as positive for lead containing paint due to the construction of the building prior to 1978. All contractor personnel working on the project shall be certified by the U.S. Environmental Protection Agency (EPA) for lead based paint activities. 7. Abatement costs will be borne by the Owner. Contractor to coordinate utility disconnections, fixture removals, and demolition prior to abatement removal with the Owner. 8. Parking; Parking on grass surfaces is strictly prohibited. Parking in driveway of the woodframe is acceptable. Off street parking is available. Additional parking is available at the Vine Street parking lot. 9. All furniture is to be protected throughout the work and cleaned at the conclusion of the project. It is the contractor’s choice to procure separate storage container for furniture storage. Clarification Note: Where it is noted in the scope of work to replace stair treads, reference is to replacement of vinyl stair treads. Attic and basement windows shall be replaced. In all locations where ceiling lighting is identified to be replaced with Incon Lighting delete reference to Incon and provide ETI Lighting as noted above. Windows 2. Basement window below bedroom #1 (northeast side of house) shall be 2/2 - tempered, low E. Remove / reinstall security bars to complete the work. Remove old dryer exhaust vent and duct in basement. Kitchen 3. Owner to hire abatement contractor for removal of kitchen flooring and countertops. Coordinate schedule with Owner. 4. Eliminate reference to Kraft Maid cabinets. Remove existing tub, mixing valve tub/shower system, drain and sliding glass door. Install new tub - Sterling Ensemble series 7137, 60"x301/8"x73". (L or R as required). All tubs shall be set in a thinset mortar bed to ensure tub floor is completely supported. 8. Install new tub/shower supply, control, drain - Symmons Temptrol s-96-2-l-x (single blade style lever handle and integral service stops). Re-install existing water saving shower head. 9. Repair and patch existing wall tile to match. 10. Remove existing sheet vinyl flooring and cove base. 11. Prep floor and install new sheet vinyl flooring and cove base. New flooring shall be installed under vanity. Caulk perimeter of sheet vinyl prior to installing cove base. 12. Remove and reinstall existing commode, vanity, sink and faucet for new floor installation. 13. Provide new wax ring, supply lines and p-trap piping for plumbing fixture re-installations. 14.
