Documents dated December 20, 2014, 5/22/2015 and May 2015 and related addenda and clarifications. Acknowledge receipt of this Addendum in the space provided on the Bid Form.

Decorative light pole base shall be DB9. Eliminate reference to DB8.
The DB9 decorative light pole base has a 3 bolt pattern. Cut sheet attached and link provided. http://www.aal.net/content/products/specs/specs_files/aal_db9_spec.pdf

A total of 6 pole bases are required.

All pole bases need to have an 8’ ground rod.

Five (5) shall have 3 conduit and one (1) shall have 4 conduit.

The pole base with 4 conduit shall be located closest to 109 Cross Street.

Coordinate with Electrical Contractor.

Please provide the proper light pole base.
A combination electrical/USB receptacle shall be placed on the pole base at 107 Cross Street,
109 Cross Street and 113 Cross Street light fixture locations.
