University College Dublin Hourly Paid Casual Work Claim Form An Hourly Paid Set-up Authorisation Form (www.ucd.ie/hr/forms) must be submitted with your first claim for this particular piece of work. If you are already set up and the end date of your original set-up has expired, a new authorisation form must be completed. Claims will not be processed without a valid set-up form.
