Annex B Checklist for Non-profit Making Organisations Phase Two of E-Textbook Market Development Scheme (EMADS) Notes for Applicants 1. One application form should be used for one subject set covering one Key Stage (KS) in one language only. If applicants wish to apply for the EMADS to develop more than one subject set or in more than one language for the same subject set, please submit separate application forms. 2. Please make sure you have submitted the following documents in your application for the EMADS. ============================================================== 1. Two certified true copies of Applicant’s relevant registration documents (including Business Registration Certificate, Certificate of Incorporation, Memorandum and Articles of Association and latest annual return obtained from the Companies Registry, whichever appropriate) 3. Two copies of a SEPARATE (i.e. not included as part of the application form) descriptive outline of the set of e-textbook delineating the curriculum objectives, content and pedagogy design of the whole subject set in accordance with the requirements as set out in the Key Learning Area curriculum/subject guide/supplementary information concerned 5. Two copies of a SEPARATE (i.e. not included as part of the application form) descriptive outline of the use of e-learning technology for the set of e-textbook with some illustrative examples, showing how to use appropriate e-learning technology for enhancing learning, a description of 1 the pedagogical rationale on why the technology solution is appropriate and a description of how the technical specifications of items (1) to (9) as outlined in Section III of the Application Form are to be implemented 6. Eight SOFT copies of a real sample (without bearing any identity of the Applicant) e-textbook chapter/module for a certain topic equivalent to about 3-4 hours of curriculum time on static storage media, such as CD, DVD, USB thumb drive, that can be accessed offline on common desktop computers. This sample chapter/module shall serve to illustrate some or all of the technical specifications as outlined in Section III of the Application Form 7. Two copies of a sample printout of e-textbook chapter/module generated by the print-on-demand function of the e-textbook for item 6 above, together with the corresponding part of the teacher’s handbook akin to the free teacher’s handbook for printed textbooks for the reference of teachers 8. The following features to facilitate navigation and search within the same set of e-textbooks shall be available. Interactive learning, teaching and assessment activities shall be provided. Input boxes / spaces and output displays / error messages shall be meaningfully labeled. Tools for annotation/note-taking, bookmarking and highlighting which are shareable and synchronisable to more than one computing device via Internet, shall be included. Highlighting and note-taking are easy to apply and remove with simple text tool box. Appropriate interface for accessing free online dictionary shall be provided. 7 No Section IV: IT Expertise Please provide information about the information technology expertise, knowledge and experience your development team possesses for the development of the set of e-textbooks. Unallowable cost items shall not be included in the development cost. Subsequent expenses thereafter relating to updating of the contents, other related upgrading or maintenance, etc., are excluded. Please refer to paragraph 4.1.2(e) of the Guide on examples of unallowable cost items. Manpower cost includes salaries and salary-related expenses (including employer’s contributions to the Mandatory Provident Fund but excluding allowances, fringe benefits, gratuity, year-end double payment, etc. of the manpower directly incurred for the development of the set of e-textbooks). In case any staff is deployed for the development of the set of e-textbooks on a pro-rata/ hourly basis, his/ her salary should be so charged accordingly. A flat rate shall be used to budget the salary of the manpower for the development of the Project. Rental/time cost of existing equipment owned by the Applicant will not be covered. Neither the Government, nor any of its officers, agents, advisors or members of the Steering Committee, accepts any liability or responsibility as to, or in relation to, the adequacy, accuracy or completeness of the information contained in the Application Form, the Guide or any other written or oral information which is, has been or will be provided or made available to any Applicant; nor do they make any representation, statement or warranty, express or implied, with respect to such information or to the information on which the Application Form or the Guide is based. Any liability in respect of any such information or any inaccuracy in the Application Form or the Guide or omission from the Application Form or the Guide is expressly disclaimed. Nothing in the Application Form, the Guide nor in any other written or oral information which is, has been or will be provided or made available to any Applicant should be relied on as a representation, statement or warranty as to the intentions, policy or action in future of the Government, its officers or agents. 2. The submission of any proposal by an Applicant shall be taken to be an acceptance of the terms of the Government Disclaimers in Section XII. 3. The invitation for submission of applications under the EMADS does not constitute any offer nor does it constitute the basis of any contract which may be concluded in relation to the EMADS or the carrying out of and completion of any e-textbook. 4. Each Applicant should make its own independent assessment of the proposed terms of the EMADS after making such investigation and consulting its own professional advisers and taking such other advice as may be prudent in order to assess the risks and benefits of the application for Government grant as well as in respect of any financial, legal, tax and other matters concerning an e-textbook project. 5. The Government reserves the right, without prior consultation or notice, to change the proposed terms of the EMADS. The Government also reserves the right to terminate any or all negotiations in its discretion before executing any binding contract with a successful Applicant. 15 Personal Information Collection Statement Purpose of Collection 1. The personal data provided in the application form will be used by the Steering Committee on the Selection, Quality Assurance and Review of the EMADS (“Steering Committee”), its relevant sub-committees, the Subject Vetting Panels, the Pricing and Implementation Panel and the EMADS Secretariat for the purpose of assessing applications to the EMADS. For successful applications, such data will also be used for project monitoring, publicity and dissemination purposes as appropriate. 2. The provision of personal data in this application form is voluntary. The lack of certain information may affect the assessment of the application. Classes of Transferees 3. Personal data provided in this application may be disclosed, if necessary, to the Education Bureau, other Government departments, expert reviewers, monitoring members of the projects and other people concerned. Access to Personal Data 4. Applicants have the right to access and correct the personal data provided in accordance with Sections 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance (Cap.486).
