Academic Advisor Feedback Template:
Your graduate student is a fellow in the NSF GK-12 grant program, Graduate Fellows in
Science and Mathematics Education. As a tool to provide your fellow with some mid-year
feedback, please write a few sentences for each of the items listed below. Then, please find
some time to discuss your comments with your fellow. Your graduate student will post your
comments electronically through in his/her ePortfolio. These comments will be reviewed
for programmatic evaluations.
1. What do you know about our program and its objectives?
2. In what ways has the GK12 program affected the professional development of your
graduate student?
3. What positive effects has being part of this grant had on your graduate student?
4. Are you aware of any negative effects that being a part of this grant has had on your
advisee? (For example, is your advisee on track to graduate?)
5. In particular, have the communication skills of your advisee changed since the
beginning of the grant experience? If so, how?
