Comprehensive Examination Eligibility Verification Form
Instructions to the Student: An important evaluative point in your doctoral work is the Comprehensive
Examination (i.e., written and oral comps). To demonstrate eligibility to take the Comprehensive Exam you will
need to submit the following documentation to your faculty advisor for review:
1. A copy of your current ADP
2. A copy of your unofficial transcripts for all course work since entering the doctoral program (from
Genisys). If you are finishing courses at the time you are requesting eligibility then include the name(s) of
the course(s) and instructor(s).
3. This Form with your name, signature, and date below.

Please note that this form must be submitted to the Program Coordinator at least seven weeks prior to the
Comprehensive Exam. It is the student’s responsibility to send all critical material to the faculty advisor for
review in a timely manner that does not necessitate an urgent faculty response.
Instructions to the Faculty Advisor: Please review the documentation that the student has provided and verify that
he or she is eligible for the Comprehensive Examination.

Copy of student’s transcript demonstrating all course work was completed satisfactorily. If the student is
currently completing coursework you will need to contact instructor(s) and obtain a current evaluation
that will function as a provisional assessment of the student’s eligibility for Comprehensive Exam.

Indicate your evaluation/review of the submitted student documentation, sign and send to the Doctoral
Program Coordinator.
