All M.F.A. in Acting students are required to participate in the end-of-year assessment/exit interview
process. Continuation in the program is by invitation. Failure to attend your end-of-year assessment may
jeopardize continued participation in the program. You will be assigned an appointment for this process.
You should complete this form and email it to your advisor by the Friday prior to your appointment.
