2. The current UMD Student Evaluation Form has been in use since spring semester
2008. If the current form was used, department heads should include the average
results of each of the five groupings of questions from the new evaluation form.
Example, spring semester 2008 and later:
Group B: Articulating Expectations and Assessing Learning (Questions 4, 5 & 6)
Range: 1 (very strongly disagree) to 6 (very strongly agree)

3. If other forms have been used, a clear, detailed summary of student evaluations of
the instructor’s presentation, adequacy and fairness of tests and grading,
availability for extra help, and overall teaching ability should be included.
4. Actual results for individual courses should be included in the archival material.
