Dear Student: The ESL Department wants you to be successful in your ESL classes and your
teacher has identified you as a student who is motivated and has the potential to succeed. The
ESL Early Alert Advisor (an ESL instructor) will meet with you for twenty minutes to help you
better understand what you can do to be a successful student.
Please make an appointment for an interview with the ESL Early Alert Advisor. Come to
Batmale 620 to sign up for an interview. Sign-up sheets will be next to the door. (Sign up early
to get the time that’s best for you.)
A. Choose an open time slot on the sign-up sheets and write your name, course and email
address. Fill in the appointment date and time at the top of this sheet. I will confirm your
interview time with you prior to the date.
B. Please bring a) this form, b) samples of your ESL work, and c) your textbooks to your
appointment in BATMALE 620. The interview will last about twenty minutes. After the
interview, you will return this signed form to your ESL instructor. Saturday appointments will
be held in a CLAD office in MUB 149.
Students in night classes and at non-Ocean campus locations: Please email me to arrange for
