CES-497 & ES-497 Report Format
Your Report Format:
Please use MS Word to write your formal technical report with 500-600 words. Include the following
format for your reports.
 No cover page to save.
 Start with the name, job title, and affiliation of the speaker, the title of the talk, date, place of
the talk, your name as the reporter, submission date of the report. Here is an example:
Subject: Report on ES Lecture series for ES497
Speaker: Dr. Robert Sargent, Director, Director of Optical Security and Performance Products, JDSU
Lecture title: "Optical Coatings and Their Applications"
Date and location of the lecture: 3/5/2015, Salazar 2009A, SSU
Reporter: John Smith
Date of Submission: 3/12/2015
 A short introduction for your report.
 The technical body, importance of the lecture, advantage (pros) / disadvantages (cons) and, esp.
applications.
 A summary paragraph of the talk, what you learned from it, and your comments.
 Include the wording of each acronym the first time you use.
 A numbered list of the references you used, at least reference to the speaker slides. Make sure
to refer to the list in the body of your report.
 It is good if you search the Internet and see if you did not understand any terminology in the
talk.
 Do not copy sentences from the papers available on the Internet. This is plagiarism. You can
only use one or two sentences provided you use quotations " " around those sentences and add
the reference. You can paraphrase important issues with reference.
 Spell-check your report before submitting.
Make sure to:
 Email your report as an attachment within two weeks after the lecture, late submissions are
not accepted.
 I will use the “Review Tool” available in Microsoft Word to review your report and respond to
you with any comments if any. If you were asked to resubmit, resubmit your corrected
report within one week.
 Please note that:
o Any report that is not accepted the first time need to be resubmitted with an extra 50 technical
words, i.e., a minimum of 550 words.
o The second resubmission should be a minimum of 650 words,
o The third resubmission should be a minimum of 800 words, and so on.
 Use the File_Name_Format below.
File Name Format: Use the following format to name the Word document of the report you submit
electronically.
 YourLastName_ES_Report_#_SpeakerLastName
Examples: If your name is “Smith” and speakers’ name is “Roberts”
Smith_ES_Report_2_Roberts.docx
 This makes it easier for me to save your files in the appropriate folders and keep track.
