SDSU Elementary Festival Honor Choir Photo Release &
Consent Form – Students Return by January 12, 2016.
My child, _______________________________, has my permission to attend the
SDSU Elementary Festival Honor Choir that is being held in conjunction with the South
Dakota In-Service Conference in Brookings, SD, February 11, 2016.
I understand that the children will be promptly chaperoned at all times and that my
child’s teacher will be present at rehearsals and the performance. I hereby waive any
and all rights for damage I may have against any organizers of the SDSU Elementary
Festival Honor Choir activity, individually, as a group, or as an organization.
Consent is hereby granted to the SDSU Elementary Festival Honor Choir for the use of
photographs, slides, and television participation involving my child with or without
association to the child’s name. These may appear in various electronic or print
publications, web pages, presentations and/or television programs that include my child.
I hereby transfer to the South Dakota State University Elementary Festival Honor Choir
all copyright and other interest in the photographs and/or video taken. I also hereby
grant royalty-free permission to reproduce in all formats including but not limited to print,
electronic, and/or CD-ROM to reproduce and include my son’s or daughter’s likeness
for the purposes of promoting the activities of the SDSU Elementary Festival Honor
Choir.
