2013 Care Coordinators’ Conference Information
A LUAU RECEPTION FOR ALL ATTENDEES WILL BE HELD MONDAY EVENING, OCTOBER 7TH, 4:30-7:00:

CONFERENCE AWARD LUNCHEON OCTOBER 8TH: Presentation of the Care Coordinator of the Year
and longevity awards.
ILLINOIS COUNCIL OF CASE COORDINATION UNITS: The membership of the Illinois Council of Case
Coordination Units is comprised of agencies that are designated as Case Coordination
Units. Case Coordination Units arrange home and community-based services for over
85,000 older adults who need assistance in order to remain independent.
PARKING: There is no parking fee.
ACCESSIBILITY: All conference sessions and activities are held in fully-accessible rooms.
CREDITS: To receive either or both of the following credits, a participant must attend the
entire two days of the conference.
You must sign-in each day to receive credit. There will be sign-in sheets at the registration
desk. CEU certificates will be emailed to you at the address you provided when you
registered for the conference.
CONTINUING EDUCATION UNITS: 9.5 hours of CEUs are available for Licensed Social
Workers and Nursing Home Administrators for attending this two-day conference.
CEU sign-in sheets will be available at the conference registration desk.
RECERTIFICATION CREDIT: Recertification Credit for Certified Community Care
Program Care Coordinators and Supervisors as mandated by Rule Section 240.1440
is available for each conference participant.
CERTIFICATES OF ATTENDANCE: The Certificate of Attendance is available at the conference
registration desk.

The Transition to Medicaid Managed Care in Illinois: An Opportunity for LongTerm Services and Supports Systems Change
Presented by Sharon Miller, Project Manager (consultant) for Illinois’ Health
Information Exchange (ILHIE/OHIT).
Ms. Miller has been responsible for interconnecting a 5 hospital network, 1500 physicians, and 14 Long
Term Care facilities on a uni-directional platform using ILHIE Direct. Project tasks include task force
organization, Project Charter, Project Plan and timeline, and overall project management to include
testing and user sign-off.

Navigating Cultural Diversity and Integration
Presented by Marta Pereyra, Executive Director, CLESE Chicago
This presentation will focus on basic overview of cultural diversity with an exercise designed to examine
individual preconceived notion on culture and values. Participants will be introduced to two communities:
Russian and South Indian and will learn about their characteristics (contrast and compare). Participants
will also understand their own perspective and how this perspective interacts with different cultures. Q&A
session with two presented populations – Russian and South Indian – will follow.

Join the presenters as they walk you through the 2014 enrollment process, give updates on Medicare,
Advantage plans, Medicare drug plans and the Affordable Care Act. Particular instructions regarding those
with Medicaid, QMB, SLIB or QI and how you can ensure they are aligned with the best choice for
insurance for 2014.
