Miller, Director of Human Resources CC: Date: June 30, 2016 Re: Worker’s Compensation Information Workers’ compensation provides benefits to employees who sustain a workplace injury by accident or illness (subject to limitations of O.C.G.A 34-9-1(4)) which arises out of and in the course of employment. The fax number to human resources is: (912) 688-6948. Ogeechee Technical College utilizes Amerisys, Inc. as its Workers’ Compensation/Managed Care Organization (MCO). Human Resources will assist the employee in filing a workers’ compensation claim and after a claim has been filed, employees can call a case manager by dialing 800-900-1582 for assistance in selecting a physician, scheduling an appointment, and obtaining follow-up care. Most importantly, employees are not free to seek medical treatment from their personal physician or a doctor of their choice unless they are approved by the Department of Administrative Services (DOAS)/Risk Management. All employees must access the network through Amerisys. Seeking treatment without the assistance of the case manager results in unauthorized care, and the employee may be held accountable for all medical expenses. The human resource office will need to know when appointments have been made and must receive any copies of discharge notices, medical notes, workplace restrictions, or prescriptions. You should not have to pay any bills up front to be seen by a doctor. If you have any problems with any doctors, please let me know. When you go to the doctor, you will need to inform the office that this should be filed under a workers’ compensation and they can our office for verification. You will need to give them your Workers’ Compensation claim number. DO NOT have any bills sent to your home or the Ogeechee Technical College address. The doctor’s office should bill workers’ compensation directly. (Refer to the Workers’ Compensation Contact Sheet). When you need to get a prescription filled, you will need to give the pharmacy some very specific information in order for them to directly bill Workers’ Compensation. (Refer to the Workers’ Compensation Contact Sheet). If you have any questions, problems, or concerns, please let me know. Except as otherwise provided in this chapter, "injury" and "personal injury" shall include the aggravation of a preexisting condition by accident arising out of and in the course of employment, but only for so long as the aggravation of the preexisting condition continues to be the cause of the disability; the preexisting condition shall no longer meet this criteria when the aggravation ceases to be the cause of the disability. "Injury" and "personal injury" shall not include injury caused by the willful act of a third person directed against an employee for reasons personal to such employee, nor shall "injury" and "personal injury" include heart disease, heart attack, the failure or occlusion of any of the coronary blood vessels, stroke, or thrombosis unless it is shown by a preponderance of competent and credible evidence, which shall include medical evidence, that any of such conditions were attributable to the performance of the usual work of employment. Alcoholism and disabilities attributable thereto shall not be deemed to be "injury" or "personal injury" by accident arising out of and in the course of employment. Drug addiction or disabilities resulting therefrom shall not be deemed to be "injury" or "personal injury" by accident arising out of and in the course of employment except when such addiction or disability resulted from the use of drugs or medicines prescribed for the treatment of the initial injury by an authorized physician. Notwithstanding any other provision of this chapter, and solely for members of the Georgia National Guard and State Defense Force, an injury arising in the course of employment shall include any injury incurred by a member of the Georgia National Guard or State Defense Force while serving on state active duty or when traveling to and from state active duty.
