The student should submit this form to the Chair of the Biology Graduate Committee. at least one month prior to receiving
the questions. If all requirements are met, the Graduate Committee will create the Panel in part based on areas of interest.
Subsequent to approval by the Graduate Committee, the form will be placed in the student's permanent file.
