If courses are articulated on the ASSIST Web site, www.assist.org, you do not need to complete this form. Please just send an email to a Nursing Advisor if we
have missed giving you credit for an articulated course in ASSIST. This form should be used for any other course equivalency petitions. Approval will be granted
for courses only in those cases where the course content and level are deemed equivalent to CSUSM courses and when taken at a regionally-accredited college
or university. The minimum acceptable grade for transfer work is a “C”. Courses may not be taken on a Credit/No Credit or Pass/Fail basis.
Please include the following documentation for each course being petitioned for equivalency: A Course Description from the Catalog for the year in which
the course was completed, or in cases where a detailed course description in not available in the Catalog, you may be required to obtain and submit a Course
Outline for the year in which the course was completed. Each institution must keep course outlines on file for all courses taught, by year and term. If the
University or the School of Nursing does not have an official transcript showing the final grade earned for each course being petitioned, you will need to have an
official transcript sent to the School of Nursing before your petition can be processed.
