This is the first such administrative review. Please follow these instructions: 1. Please answer all questions using the scantron form provided. Use a pencil to answer the questions. Please do not add your name or any other identifier. 2. Please provide written comments on a separate sheet of paper. 3. Place your scantron form and written comments in the small, unmarked envelope and seal. 4. Place the small envelope in the larger envelope addressed to Barbara Leonard, Chair, Faculty Congress, College of Business and Economics and sign and print your name and department on the outside of the envelope. Blank envelopes will be discarded. Send via campus mail by … Faculty Congress will have a minimum of three Congress members overseeing the collection of data. All names on envelopes will be checked against a master list of names, and then the inner (blank) envelope will be placed in a box with all the other blank envelopes. The blank envelopes will be opened and compiled under supervision of at least three Congress members. The results will be reported at the April Faculty Congress meeting in Executive Session and a written summary report will be sent to the UH Hilo Chancellor and UH President. Each Vice Chancellor will be sent a summary of their own review. The results will not be reported in the minutes, and will be held in confidence by the members of Faculty Congress.
