Thesis candidate MUST complete this form and submit it to the Honors Program no
later than three weeks before graduation in the semester the student plans to graduate.
Completing this form only indicates intent to finish the thesis and graduate. If the
student does not finish the thesis and/or does not graduate, he or she must notify the
Honors Program and then resubmit this form again in the semester graduation is
planned. (Final grades will not be recorded until the official grade sheet is received by
the Honors Program.)
Submission of this form is necessary before any incompletes can be removed and before
the student’s names will be submitted to the graduation office.
