INTERNSHIP MID-TERM EVALUATION
(to be completed by the Agency Supervisor)
The Internship Mid-Term Evaluation provides a description of the student’s
performance up to the midpoint of the Internship. It also provides the student with an
assessment of his or her abilities and overall performance and suggests areas for
improvement. The Agency Supervisor should complete this form the third or fourth of
the internship (not the last minute). After discussing the evaluation with the student, the
student should sign and forward it to his or her Faculty Supervisor.
