1.
Turn in ONE “main file” (all documents except the appendices) in a binder. Label the binder “Original, Faculty Name”. You do NOT have to submit a hard copy of the appendices, however please do submit books or creative works (such as CDs) which cannot be scanned into a PDF for the research appendix.

Turn in a copy of the case on a writable CD or memory key. Include the following information.
● PDF of the “main file” (all documents minus appendices)
● PDF of the research appendix
● PDF of the teaching appendix
● Word or Excel file of the Document Register
Note: The Dean’s Office will update the Document Register and the Summary of Actions sheet when the CAS P&T Committee minutes and Dean’s recommendation are added to the file. Any other additions to the file will also be recorded before the case is sent to the Provost’s Office.
3.
Make sure each document is labeled. Documents with two or more pages can be stapled together.

CAS has bankers’ boxes available to package each case, if needed (if submitting items in addition to the main file).
