As proof of eligibility to access this funding, I hereby declare the following to be true and correct:
I am an existing worker in the building and construction industry, with a minimum one month or more working
relationship with my employer _______________________________________
AND
I am an eligible worker as defined in the Building and Construction Industry (Portable Long Services Leave) Act
1991 (Qld)
AND
I am an Australian or New Zealand citizen OR a permanent resident of Australia OR a refugee and humanitarian
visa holder. (Please circle)
AND
I am a permanent resident of Queensland OR I am permanently employed in Queensland. (Please circle)
I further declare that:
I am not an employee of any Authority
I am not currently enrolled and participating in a school program.
I am not a contracted trainer or assessor or existing worker of any RTO.
I am not already funded by an Authority or any other entity for delivery of the same training being undertaken as part
of this Program.
I am not previously funded under this Program in the same contract period (from 1 October 2015).
I have attached copies of:
(i) One (1) of the following (please circle):
Medicare card (Green only) OR Full Birth Certificate or Birth Certificate Extract
OR
Passport
OR
AND
(ii) One (1) of the following (please circle):
Driver Licence OR Telephone account OR Bank Statement OR Statement from a real estate agent
OR Any other formal documentation stating my full name and current residential address.
AND
(iii) Evidence of employment status and occupation (if relevant) e.g. letter from employer or payslip.

Visa
I understand that training under this program is subsidised by CSQ as a contribution to the cost of services to reduce the
cost to participants. The funding allocation is provided to the RTO (Institute of Management) on successful completion of
each unit of competency.
A participant fee is payable on enrolment to Institute of Management to cover the balance of costs for delivery of the
qualification.
As a recipient of funded training I consent to CSQ contacting me for the purposes of, but not limited to, a review and
conducting destination surveys.
