Service plan is a generic term that covers the individual or group plans (which may also be referred to by other names) that are developed by service providers for people receiving support (and may include their family/whānau as appropriate). Evidence requirements 1.1 Life roles are described in terms of family/whānau structures, responsibilities, and inter-personal relationships. Range evidence is required for a minimum of four life roles. 1.2 The effects of brain injury on a person's life roles are described in terms of the person's strengths, participation in work and/or study readiness, and barriers to participation. 1.3 Risks are identified in terms of a person's ability to maintain relationships and life roles after brain injury. Range evidence is required for four risks. Outcome 2 Describe strategies to support a person after brain injury to fulfil his or her chosen life roles. Evidence requirements 2.1 Strategies to support a person after brain injury to fulfil his or her chosen life roles are described in accordance with the person’s service plan. Range evidence is required for three strategies. Outcome 3 Implement strategies to support a person after brain injury to fulfil his or her chosen life roles. Evidence requirements 3.1 Strategies to support a person after brain injury to fulfil his or her chosen life roles are implemented in accordance with the person’s service plan.
