Special Use Permit Checklist Fraternities & Sororities Please use the following checklist to ensure your packet is not missing any information. Upon submission, the Code Enforcement Office will verify that all items on the checklist are included. If you have any questions about developing your special use permit application please call the Village Code Enforcement Office at 7462622. Incomplete applications will not be considered by the Village until such time all materials as required have been submitted. Planning Board meets the first Tuesday of every month. Applications are to be submitted two weeks prior to meetings. Items Required Special Use Permit (June 1st thru May 31st) $100.00 non-refundable application fee with check payable to the Village of Delhi Clerk Provide a valid and current Certificate of Substantial Compliance issued by the Code Enforcement Office Complete list of officers (name, phone, e-mail, mailing address) Position descriptions for all officers (include person responsible for risk management, RA, house manager, fire safety officer) Complete directory of all members residing in house List of all alumni corporation members and faculty/staff advisors Contact information of owner of chapter house (name, phone number, date of birth, address) Contact information for local representative if owner is NOT a local representative Copy of chapter and house policies specific to house (what is given to members at the beginning of each year) Clear and detailed copy of fire safety policies (include emergency procedures, responsibilities of on-site fire safety) Copy of policies and plans for dealing with alcohol consumption in chapter facility (risk management) Copy of policies regarding property maintenance (trash removal, grounds keeping and snow removal, etc.) Proof of coverage of housing insurance Occupancy limits by number of people for recreational and assembly uses of chapter house Documentation of any construction or changes in building layout or structure since the last special use permit was issued Documentation of any changes or construction planned for the next year Documentation of any changes in fire safety elements for the next year Copy of letter that was sent to neighbors within 500 feet of the property A summary of the chapter involvement on the campus and in the community (i.e. community service activities, leadership on campus, etc. Letters of recommendation (not mandatory but helpful) Chapter Use Code Use
