THIS AMENDMENT NO. __ to the University Subcontract is made and entered into by and
between THE BOARD OF TRUSTEES OF SOUTHERN ILLINOIS UNIVERSITY, a body politic and
corporate of the state of Illinois, whose principal office is at Carbondale, Illinois, by and on behalf of its
School of Medicine, hereinafter referred to as “University” and
