The final dose of polio vaccine given prior to August 7, 2009 will fall under the previous recommendation with a minimum interval of four weeks between doses three and four. If given before, dose is not valid and must be repeated. Varicella and MMR must be given on the same day if both are needed, or separated by at least 28 days. Hepatitis B Student needs three doses prior to Kindergarten. 1st dose* 2nd dose At least 4 weeks apart 3rd dose** Must meet all three criteria** *First dose can be given at birth. **3rd dose must meet the following – (1) Student was a minimum of 24 weeks of age, (2) A minimum of eight weeks have passed after receiving the second dose and (3) the minimum interval between dose one and dose three is 16 weeks. Varicella (Chickenpox) Must have received two doses of vaccine before kindergarten or 7th grade (starting in 2015-2016) or have a history of disease**. 1st dose* 2nd dose At least 4 weeks apart required prior to Kindergarten and 7th grade 2015-2016 *First dose must be administered on or after the student’s first birthday. If given before, dose is not valid and must be repeated. If student is 13 years old or older at the first dose needs two doses 4 weeks apart. Varicella and MMR must be given on the same day if both are needed, or separated by at least 28 days. **If a student has a history of Chickenpox the parent/guardian must sign the Utah School Immunization Record stating the student had the disease. Hepatitis A Student needs two doses prior to Kindergarten. 1st dose* 2nd dose At least 6 months apart from very last dose even if dose was invalid. * First dose must be administered on or after the student’s first birthday. Meningococcal Student needs one dose prior to 7th grade. 1 dose* *Generally given at 11-12 years of age.
