Student needs 5 doses by kindergarten and a Tdap before entering 7th grade

A student who has received 6 or more doses before their seventh birthday does not require more doses prior to kindergarten regardless of spacing.
Unless they received a Tdap after age 7 will need a Tdap booster prior to 7th grade.
Call for assistance with students over 7 years old who are incomplete.
Polio
Student needs 4 doses unless the third dose is administered on or after the students fourth birthday, then the fourth dose is not required.

**If third shot is given on or after the students fourth birthday AND at least six months after the previous dose then the fourth dose is not required.
*** The final dose of polio vaccine given on or after August 7, 2009 must be given at a minimum age of four years AND a minimum interval of six
months following the previous dose. The final dose of polio vaccine given prior to August 7, 2009 will fall under the previous recommendation with a
minimum interval of four weeks between doses three and four.
MMR
Student needs 2 shots by kindergarten

Varicella and MMR must be given on the same day if both are needed, or separated by at least 28 days.
Hepatitis B
Student needs three doses prior to Kindergarten.

**3rd dose must meet the following – (1) Student was a minimum of 24 weeks of age, (2) A minimum of eight weeks have passed after receiving the
second dose and (3) the minimum interval between dose one and dose three is 16 weeks.
Varicella (Chickenpox)
Must have received two doses of vaccine before kindergarten or 7th grade (starting in 2015-2016) or have a history of disease**.

**If a student has a history of Chickenpox the parent/guardian must sign the Utah School Immunization Record stating the student had the disease.
Hepatitis A
Student needs two doses prior to Kindergarten.

Meningococcal

*Generally given at 11-12 years of age. A booster dose at age 16 is recommended
