GRANT ASSURANCES
FISCAL YEAR 2012 (2011-2012)
The lead applicant agrees to all of the following (each one must be checked to indicate
agreement):
______ Work collaboratively with partners to implement grant activities as described in this proposal;
______ Work collaboratively with partners to ensure the participation of students with disabilities and
English language learners in an inclusive setting;
______ Ensure that children/youth are not prevented from participating in the After-School and Out-ofSchool Time (ASOST) Quality Grant programs/services offered because their families are unable
to afford tuition. Program must use a sliding fee scale to support access for all children if tuition is
charged;
______ Evaluate program services and activities to determine effectiveness; and
______ Submit required financial and programmatic reports, including a post-program report on program
outcomes and evaluation activities. Details and guidelines will be provided to all grantees.
